For all users:

• I understand and agree to indemnify and hold harmless Luminara Academy, its affiliated sites, staff, and employees from any liability concerning my child’s involvement in Luminara Academy’s programs. I acknowledge that participation in the Academy’s activities is at my child’s own risk.
• I understand and agree that any photographs or videos taken by Luminara Academy during program activities can be used in any promotional material or advertisement for the Academy. This includes, but is not limited to, use in brochures, posters, newsletters, and digital media.
• The parents/guardians affirm that the participant(s) is/are of the appropriate age for the program(s) in which they are enrolled.
• By agreeing to this form, parents, guardians, and participants agree to abide by all rules, regulations, guidelines and procedures of Luminara Academy, as outlined on the website.
• I confirm that I have read and understood the Registration and Enrolment Agreement on the course pages.
